Illinois Smog Check Requirements to Register Car

Only residents of the following counties need to comply with the state's emissions testing standards: Cook, Lake, Dupage and parts of St. Clair, Kendall, Kane, Madison, Monroe, Will, and McHenry. If you do not live in any of these designated areas you're free from smog check obligations. 

You do not need to provide proof of passing an emissions test during the initial car registration process. It's only during registration renewal when the state enforces its smog inspection requirements. You'll be reminded of this obligation in your registration renewal notice. The Illinois EPA will also send you an emissions test notice approximately four months prior to the expiration date of your current vehicle registration. This will give you plenty of time to comply, especially if your vehicle fails the emissions test and requires repairs. 

If the state does not have proof of your vehicle passing a smog check, you will not be allowed to renew your car registration. And if you're caught driving with an expired vehicle registration, you will be fined, ticketed, and possibly face license suspension.
