Replacing Vehicle Titles in Pennsylvania

The Pennsylvania Department of Transportation (PennDOT) is the one stop location to visit if you need to replace a car title. Only the owner of the car can receive a duplicate title for a damaged or lost title in Pennsylvania (PA).

For example, if the car is financed and the loan has not been paid off yet, the lienholder has to apply for the duplicate title. If the car is paid off, you can apply to receive a duplicate title as the vehicle owner.

To request a duplicate title:

Complete the "Application for Duplicate Certificate of Title by Owner" (Form MV-380). Data that you or the lienholder will provide on the application includes:

  • Vehicle identification number.
  • Title number.
  • Your or the lienholder’s name and street mailing address.
If the loan is paid off, ask the lienholder to complete the bottom portion of the application. This is where the lienholder will input the date that you paid off the loan.

The title transfer application is required to be signed by you (the buyer) and the lien holder.

Send the completed and notarized application to PennDOT with the $22.50 title fee. The address to send the form to is:
