From a purely legal standpoint, without passing a car inspection you cannot renew your vehicle's registration. And without an updated registration you cannot legally operate the car in question.

From a safety standpoint, advocates of the state inspection system claim it saves lives by keeping cars with say, faulty brakes or broken windshield wipers, from operating on the roads and endangering other drivers. Some safety experts claim that 12 to 33 percent of all accidents can be blamed on poorly maintained vehicles.

As is, only 19 states require annual or biennial car safety inspections as a prerequisite for vehicle registration.
