Submitting Your CDL Medical Exam Report

To submit a medical form for a NY commercial license, you must complete an Application for Driver License and Non-Driver ID Card (Form MV-44) and submit with it a photocopy of your Medical Examiner Certificate. This certificate must be current (issued over the last 24 months).
 
NOTE: Each CDL driver must renew the Medical Examiner Certificate every two years and carry this document with him or her at all times when driving.
