Medical Exam Report Requirements

When applying for a CDL you will be required to submit a medical form to the DMV. It must be completed by you and a licensed physician.

Based on the federal requirements of the Interstate Commerce Commission, the North Carolina Division of Motor Vehicles and NC Department of Human Resources have adopted the following medical requirements for commercial license drivers:

Vision


  • Visual Acuity: Must have 20/40 vision for each eye or both eyes together, with or without glasses.
  • Field of Vision: Must have at least 70 degrees in the horizontal median in each eye.
  • Color Vision: Must be able to distinguish the colors red, green, and yellow.

Hearing


You must have good enough hearing to detect warning sounds made by car horns, emergency vehicles, railroad crossing signals, and train whistles. NC considers the ability to hear a whispered voice from five feet as adequate.

Illness


  • You cannot have any medical history or clinical diagnosis of diabetes mellitus currently requiring insulin for control.
  • You cannot have a current clinical diagnosis of myocardial infarction, angina pectoris, coronary insufficiency, thrombosis, or any other cardiovascular condition known to be accompanied by syncope, dyspnea, collapse, or congestive cardiac failure.
  • You cannot have any medical history or respiratory dysfunction that could interfere with your ability to drive.
  • You cannot have been recently diagnosed with high blood pressure that could hamper your ability to drive safely.
  • You cannot have a medical history of rheumatic, arthritic, orthopedic, muscular neuromuscular, or vascular disease that could interfere with your ability to drive.
  • You cannot have a medical history of epilepsy or any other disorder that could cause blackouts, seizures or loss of consciousness.
  • You cannot have a mental or psychiatric condition that could hamper your ability to drive safely.

Physical Condition


  • You cannot have loss of leg, foot, hand, or arm. If you are missing a limb, you'll need to provide a skills performance evaluation certificate from the Federal Motor Carrier Safety Administration.   
  • You cannot have impairment of finger or hand, preventing you from grasping.
  • You cannot have impairment of foot or leg, preventing you from properly operating a vehicle.

Other CDL Requirements

Each CDL driver is also required to pass a general knowledge CDL test. All of the questions are based on information found in the North Carolina CDL manual. Study this manual and you should have no problem passing the test on your first try. You can obtain a manual from any DMV office, or by downloading it from the DMV's website.

You should also take a CDL practice test. These exams provide questions similar to those on the actual test, giving you a good idea of what to expect. You can find CDL practice exams online from a variety of independent sites.
