Replacing Lost ID in Illinois

The Illinois Secretary of State (SOS) makes it easy to replace a lost, stolen, or damaged identification card.  To apply for a duplicate card, visit your local driver services facility with the following:

  • One major form of identification, proving your signature. This may include a canceled check from the last 90 days, a major credit card, or a driver education certificate (this works well for teens).
  • One other form of ID proving either your date of birth, Social Security number, or IL residency.
  • Proper documentation if you're replacing your state ID due to a name change. Depending on the circumstance this may include a court order, a divorce certificate, or a marriage certificate.
  • Cash, or a check, money order, or credit card to pay the $20 duplicate card fee, or $10 if you're younger than 18.
If you're a teen, making sure you get your duplicate ID is important, especially if you don't own a drivers license. As you get older you'll find that possessing photo ID represents freedom. With an ID card you can cash checks, register to vote, and checkin at airports.
