In order to obtain a duplicate car registration certificate in LA, simply mail the following to your local Office of Motor Vehicles (OMV):

- A written request for duplicate registration, to include the Vehicle Identification Number (VIN), license plate number, proof of liability insurance, and the vehicle’s make and model.
- $12 duplicate vehicle registration fee, in the form of a check or money order, payable to the Department of Public Safety (DPS).

You can apply for a duplicate title at the OMV, by submitting the standard $18.50 duplicate title fee, in addition to an $8 handling fee.
