Getting a Duplicate NH Car Registration

Go to the DMV site and download form RMDV 110 - Application for Copy of Registration. After you've completed and signed the short form, submit it to the DMV office in Concord (the address is listed on the form). Or, take the form to any registration office or municipal agent office.
