Replace NH License Plates/Car Tags/Decals

Start by downloading form RMDV 125 (Application for Replacement Plates and/or Decals) from the DMV site. Be sure to complete the entire form, and indicate why you're requesting a replacement. You must also notify your local law enforcement office about this.

After completing the form, either submit it to the Concord office or a registration office. Decals cost $4, while each license plate costs $4.

You'll receive a temporary permit and paper plate to use until your official plates arrive. Remember, you shouldn't drive with missing plates or decals; you risk getting a traffic ticket and fine for doing so.
