How do I replace lost or damaged OR license plates, stickers, tabs or tags? Oregon
You can replace lost Oregon license plates and car tags by submitting the following to DMV headquarters:

- Completed Form 268, the Application for Registration, Renewal, Replacement or Transfer of Plates and/or Stickers.
- $23 license plate fee.
- $10 replacement fee.

Replacement License Plates for Non-Passenger Vehicles
If you need to replace a license plate for a non-passenger vehicle, contact DMV headquarters directly at (503) 945-5000. If you live in the Portland area, call (503) 299-9999.Replacement Vehicle Registration
If your car registration has been lost or damaged, you can obtain a duplicate registration certificate by submitting the following to DMV headquarters:- Completed Form 268.
- $5 replacement fee.
