You can replace lost Oregon license plates and car tags by submitting the following to DMV headquarters:

  • Completed Form 268, the Application for Registration, Renewal, Replacement or Transfer of Plates and/or Stickers.
  • $23 license plate fee.
  • $10 replacement fee.
