The state of Oregon issues two license plates for passenger vehicles, both of which must be clearly displayed. You can obtain personalized plates by submitting the following to the DMV:

  • Completed Form 268, Application for Registration, Renewal, Replacement or Transfer of Plates and/or Stickers.
  • Completed Form 205, Custom Plate Application.
  • $5 fee for two plates, or $3 for one plate.
  • Registration renewal fees, if applicable.
You can apply for vanity plates in person at a local DMV branch, or by mailing all documents and fees to the following address:
