Applying for a RI Duplicate Vehicle Registration Card

Start by downloading and completing form TR-1 from the DMV site. Make sure to fill in the appropriate replacement card sections, as listed on the top of the form. Include your insurance information, too.

Then, sign the form in front of a notary public, and mail it along with your $18.50 check as payment for the registration card fee to:
