UT Duplicate Title Procedure

To replace a damaged or lost title, download the aptly named Duplicate Title Application (form TC-123) from the DMV site.

You can then drop off the completed form at a DMV branch office and pay the $6 fee. You don't need to produce the bill of sale or vehicle registration.

Two important notes, though. Only the car title owner may sign the form. And, if there's a lien on the vehicle, you may need to get lienholder's approval for the duplicate request.
