Apply for a Duplicate Title in VT

You'll need to apply in person at the main DMV office in Montpelier. When you arrive, have:

  • A completed Affidavit of Loss of Certificate of Title and Application for Duplicate (DMV form TA-VT-16). All owners listed on the car title must sign the application, unless the names are separated by the word "OR." Only one owner must be present when applying for a duplicate title. 
  • A lien release or releases, if applicable. If a lien still exists, the lienholder must submit the application.
  • Proper identification, such as your drivers license and/or car registration
  • Payment for $31 duplicate title fee. 

NOTE: Duplicate titles will not be issued for vehicles more than 15 years old.
