If necessary, you can request the form by calling (800) 642-9066 and; of course, you can visit your local WV DMV Regional Office to complete the entire transaction.
- Print and complete Form DMV-32-TR, the Application for Duplicate Registration Card, Decal, or Plate & Change of Address form. (If there are two people joined by “AND” on the vehicle’s title, both of you must sign the application.)
- Check the box beside “Duplicate Registration Card.”
- Write a check or money order for the $5 replacement fee.
