Ordering a duplicate vehicle registration is easy. Simply:

  1. Print and complete Form DMV-32-TR, the Application for Duplicate Registration Card, Decal, or Plate & Change of Address form. (If there are two people joined by “AND” on the vehicle’s title, both of you must sign the application.)
  2. Check the box beside “Duplicate Registration Card.”
  3. Write a check or money order for the $5 replacement fee.
  4. Mail everything to:
