Your car title is one of your vehicle’s most important documents. Without it, you can’t complete a vehicle registration, title transfer, or anything in between. Fortunately, the WV DMV makes it easy to replace a destroyed, defaced, or lost title.

Obtaining a West Virginia Duplicate Title

To replace your title:

  1. Print and complete Form DMV-4-TR, the Affidavit of Duplicate Title for a Vehicle or Watercraft.
  2. Make a photocopy of your drivers license. You won’t receive your duplicate title if this photocopy isn’t included with the application.
  3. Write a check or money order to cover the $10 replacement title fee.
  4. Mail everything to:

Unlike other transactions involving titles, you do not need a bill of sale to obtain a duplicate.
