Applying for a WI Duplicate Title

Start by downloading the Title/License Plate Application (Form MV1). Be sure to complete the applicable sections. The car owner must sign the form.

When the application is filled out, the title owner should drop off the form at a DMV service center for processing. The DMV will ask for appropriate proof of identity (Wisconsin drivers license or state ID card). The bill of sale doesn't need to be shown.

If it's not possible for the title owner to come to the office, the replacement title will be mailed to the owner. Or, someone else can drop off the application and get the title as long as a completed Permission to Pick Up Title Application (Form MV2932) and proper identification are presented.
