WI Personalized Plates Process

To start, download and complete the Personalized License Plates Application (Form MV2428).

Be sure to include your preferred combination, which can include up to seven characters (numbers and letters). You also must give a brief explanation about the reason behind your combination request. (Even if it's an obvious choice, like GOPACK.)

Then, mail the form with your check or money order (payable to Registration Fee Trust) to:

Keep in mind your requested license plate combination(s) can be denied if it's already in use, or the DMV deems it inappropriate. Generally, it takes four-to-six weeks to manufacture your license plates after your request is approved.

If necessary, you can apply for a temporary license plate at a DMV service center or authorized third-party location. You may use the plate until your permanent custom license plate arrives.
