Most states require car insurance companies to submit an SR22 form for each motor vehicle that they provide liability auto insurance coverage for. Reasons that your Division of Motor Vehicles (DMV) office might require you to submit an updated SR 22 are:

  • You are convicted of driving under the influence (DUI).
  • You received a license suspension.
  • Excessive points are recorded on your drivers license (due to traffic accidents, etc.).
  • You were previously convicted of driving without car liability insurance.

SR22 Filing Requirements

If your DMV office mandates that you show insurance proof before you register your car or truck or renew your drivers license, contact your insurance company and ask them to mail you a completed SR22. File the form with your DMV office in person.  Your auto insurance company can also mail the form into the DMV office on your behalf.

Your name, street mailing address, and car insurance policy number are required to be on the SR 22 insurance form. The name of the insurance company and their street mailing address must also be included on the form.
