Unfortunately, there isn't a single answer to that question; what you need to submit depends on your traffic court and/or state driver's license department.

In most cases, you probably won't need to present a copy of your driving record as your state already has this information. But, it doesn't hurt to submit a copy if you're unsure whether you're required to do so. You may be able to order one through your online traffic school. If not, you can order one through your state's driver's license division or a third-party provider for a small fee.

Perhaps more importantly, though, is the matter of your city traffic school certificate. Find out if you must submit it yourself, or if the school will do it for you. Some courts or state driver's license departments require the certificate to be directly submitted from the school.
