Each state has different learners permit requirements. The grid below lists what documents are needed to apply. Keep in mind that the grid applies to teen drivers under 18, unless otherwise noted. Depending on your state, parental consent requirements are either satisfied by a separate affidavit or a signature on the permit application. Some states also require parental accompaniment during the application process. Check with your state's DMV for exact specifics.
